What is a penalty sheet?

A penalty sheet (lookup sheet) is a document issued by the Department of Buildings that outlines how many elevator devices are out of compliance at a particular building. Penalty sheets are required to pay the total fine amount due for administratively issued elevator violations (EVCAT1, ACC1, VCAT1, LL 10/81) issued before 2014.

To obtain a penalty sheet, request one via e-mail for your specific building (elelookup@buildings.nyc.gov. You can also reach out to the Elevator Division at 212 393-2144

As of June 2020, you should be able to view specific fines for any "failure to file" elevator violations in DOB NOW, without requesting a penalty sheet. See SiteCompli's Blog Post for more information.
